Location: Based at Queen Elizabeth Hospital SE18 4HQ

Job Term and Hours per Week: 12 month Fixed Term Contract / Secondment (subject to approval); 35 hours per week

We are looking to recruit to a newly established post to triage clients for the Discharge to Assess bedded units in line with the eligibility criteria and to ensure oversight and flow to and from the units.

You should be experienced at completing Strength based, person centred Care Act assessments, commissioning care/placements and liaising with clients, families, and other professionals/agencies. 

You should have the ability and skills to work collaboratively with other directorates and external partners dealing with issues around hoarding, pest control, Blitz cleans and repairs to enable safe return home or to alternative accommodation.

We are seeking a worker with experience of hospital discharge, ability to complete written work to a high standard whilst working in a fast-paced environment ensuring safe and timely discharges both from hospital or a Discharge to Assess Unit.  This is a client facing role so the candidate will need good interpersonal skills and the confidence to work collaboratively with a multi-disciplinary team.   You will be able to deal with complex situations and family dynamics, while always advocating for your Service user and promoting independence wherever possible.

The successful candidate will need a good knowledge of the issues facing older people and people with disabilities and an understanding of the legislative framework pertaining to Adult Social Care and hospital discharge.  This role will also include a good knowledge of issues related to possible abuse and be experienced working within the Safeguarding Adults legislation and being able to complete concerns/enquiries as necessary.    

This role will supervise/support up to three other workers and will include working with Commissioning Colleagues regarding developing any new provision or raising concerns/suggestions to improve process and pathway to and from the units.

You should be an organised team worker who is committed to the delivery of excellent services to the residents of the Royal Borough of Greenwich.
