Job Title: Reablement Support Worker 

 

Overview: The Royal Borough of Greenwich is seeking a dedicated Reablement Support Worker to join our health and social care team. This is a fantastic opportunity to make a positive impact on individuals’ lives, supporting them as they regain their independence following hospital discharge or health setbacks. Reablement Support Workers play a crucial role in helping people relearn or adapt the skills they need to care for themselves and live safely in their own homes. 

Key Responsibilities: 

  • Assist with Daily Living Skills: Provide support to clients with daily activities such as cooking, cleaning, and managing medication.
  • Mobility Support: Help clients with mobility tasks, including getting in and out of bed, using stairs, and walking around. Moving and Handling equipment.
  • Personal Care Assistance: Support with personal care activities such as bathing, dressing, and eating, ensuring clients feel comfortable and dignified.
  • Encourage Social Engagement: Facilitate social activities by helping clients get out and about, meet friends, and participate in hobbies.
  • Goal-Oriented Support: Work with clients who have set SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals, following a personalized care plan to help them achieve independence.
  • Provide Encouragement: Offer emotional support and motivation, helping clients overcome challenges and build confidence in their abilities.
  • Monitor Progress: Track the progress of clients in reaching their goals, adjusting support where necessary.

Key Skills & Qualifications: 

  • Experience in health and social care, preferably in reablement or a similar role.
  • Ability to support clients with a wide range of personal and domestic activities.
  • Understanding of the importance of maintaining independence for clients.
  • Compassionate, patient, and encouraging attitude toward individuals facing health challenges.
  • Good communication skills to engage effectively with clients and team members.
  • Flexibility and ability to work as part of a team.
