Job Introduction
Level 2 position, based in the Complex Care Team.
We are excited to be able to offer a level 2 social worker position based in Complex Care Team, based in The Woolwich Centre reporting to an Deputy Team Manager.
As a level 2 social worker, you will be required to take an active part in supporting the team, working closely alongside health and social care colleagues. This is a key post within the team and the ideal candidate must have recent experience of working within a complex care service. You will need to have experience in supporting staff and as a duty worker role be able to manage conflicting priorities, provide advice and guidance to reduce risk and maintain client safety wherever possible and ensure strength based approach. The candidate will need to be able to provide support and guidance in case discussions and be able to agree and sign off completed work by staff and ensure cost effective care. This role also requires regular liaison with other health and social care professionals as well as colleagues in housing environmental health and health providers.
A crucial part of this role will be to work closely with Commissioning, RBG, Oxleas and the CCG to ensure safe and effective pathways for the future, and to identify opportunities for us to continue to improve the outcomes we achieve for our residents. This will offer the successful candidate the opportunity to enhance their professional development within the wider context of social care provision and contribute to national initiatives.
We are looking for someone who wishes to progress and develop their skills and knowledge cross practice and management. You will understand and develops the interface between social care and health, has some knowledge of the wider system pressures and has experience of working alongside health partners. They must be motivated, passionate about excellent service delivery and be interested in building a strong social care framework.
You will hold a case load and expected to supervise social workers and or social care assessors and take a lead in an area of interest as decided by the team manager.
